Health Care Transitions

Introduction

This set of tools helps health care providers, families and caregivers supporting youth with intellectual and developmental disabilities who are transitioning from the child to the adult health care system. The Transition Checklist prepares families and youth for a discussion with their health care provider around skills and supports needed for successful transition. The Transition Plan guides health care providers to set goals for transition planning with the patient and their families. Finally, the Transfer Tool helps family physicians identify and address potential gaps in planning the health care transition.

headshot of megan henze Megan Henze is an occupational therapist with experience working with adolescents and young adults with disabilities as they transition into adulthood. At the Developmental Disabilities Primary Care Program, Megan has collaborated with experts at St. Michael’s Hospital (Toronto) to establish a transition clinic with a focus on transition-aged youth with intellectual and developmental disabilities. She previously worked in the Good 2 Go Transition Program at the Hospital for Sick Children, Toronto. Her work in Good 2 Go focused on assisting patients and families with the development of self-management skills and transition planning for health care, funding, services, employment and housing. Megan has a special interest in factors that assist adults with disabilities to live in community-based settings with rather than institutional settings.
