Understanding today’s visit and follow-up

Introduction

This tool is for people with intellectual and developmental disabilities to complete with health care providers to foster understanding of the purpose, and outcome of today’s health care visit. This could ideally serve as documentation of a health care visit for the person’s record, if needed.

NEW! This tool is now available as an e-form, integrated with a patient’s electronic health record. Patients can fill in the form on their devices (e.g., laptop, iPad) before a health visit, and the information will be shared directly in a secure way with the doctor’s office. Ask if your doctor uses the Ocean communication platform and alert them to the electronic version of this form. Physicians can access the e-forms via the Forms Library on OceanMD.

Using My Health Care Visit when going to the doctor

Dr. Yona Lunsky is Director of the Azrieli Adult Neurodevelopmental Centre, and Senior Scientist at the Centre for Addiction and Mental Health. She directs the Health Care Access and Developmental Disabilities Program (H-CARDD) which brings research, policy and practice together to improve the health of adults with developmental disabilities. She is a Professor in the Department of Psychiatry at the University of Toronto and Adjunct Scientist at the Institute for Clinical Evaluative Sciences (ICES), Toronto.

Dr. Lunsky’s research explores clinical and systems issues related to health services. She is involved in several projects focused on improving primary, emergency and mental health care and is particularly interested in working together with health care providers and recipients to design tools to improve health care delivery.

My Health Care Visit was developed through the Nuts and Bolts project, a partnership with the Centre for Addiction and Mental Health and Vita Community Living Services/Mens Sana, Toronto. It was pilot tested at Vita Community Living Services with the generous input from two consumer self-advocate groups, families, and staff working in this sector.

This is an update to a previous version of this tool (2011), published as Today’s Visit In: Sullivan WF, Developmental Disabilities Primary Care Initiative Scientific and Editorial Staff, editors. Tools for the primary care of people with developmental disabilities. Toronto: MUMS Guideline Clearing House; 2011, p. 24-25.
