Introduction

The Getting Ready For My Shot resource includes vaccination preparation tips and forms for caregivers and the person they care for. People with intellectual and developmental disabilities might be uncomfortable with injections or reject medical procedures based on previous negative experiences or fear of needles. Good preparation can help to make sure that a vaccination causes minimal distress. The Getting Ready For My Shot forms can be used to ask for specific accommodations at the vaccination appointment.

This tool is developed in response to the COVID-19 Pandemic by the Developmental Disabilities Primary Care Program of Surrey Place, Toronto, 2021. The content of this tool was subject to review by Surrey Place clinicians, primary care providers and other relevant stakeholders.
