Tool Objective

This brief one-page form should be filled out by a caregiver and primary care provider in anticipation of a person with intellectual and developmental disabilities being seen in an Emergency Department. It provides a summary of key information the Emergency Department staff will need when caring for the person with intellectual and developmental disabilities.

The Developmental Disabilities Primary Care Program is currently in the process of updating this 2011 practice resource to reflect the Primary care of adults with intellectual and developmental disabilities: 2018 Canadian consensus guidelines (Canadian Family Physician, Vol 64: April 2018). Updated practice resources are scheduled to be launched in October 2018.
